Your
proposal is a critical document in securing the funding for your
grant, and must include these important elements:
Cover
Letter:
Include a brief description of your organization and of your proposed
program. Include the amount of funds that you are requesting,
and the time frame of the distribution. Include the organizations
legal name (as it appears on its IRS exemption letter), address
and telephone number, also the name and address of the appropriate
contact person.
Background
information:
History and purpose of your organization, and the people and groups
your organization serves or will serve.
Specific Request:
Description of the proposed program and its objectives along with
evidence showing a need for the program and its value. Also include
the geographic area to be serves. Include an outline of the program
phases, from beginning to end. Include the anticipated methods of
evaluation.
Personal Information:
Include the organizations personnel who will implement the program
and their qualifications. Also, include the names and principal
affiliations of directors, senior staff and trustees.
Financial Information:
Include a detailed program budget showing other sources of funding
and an overall organizational budget showing other sources of funding.
Include an overall organizational budget covering the years during
which the proposed program will take place. Include the organizations
most recent annual financial statement, (preferably audited). A
statement of the sources of organization funds, both public and
private.
Addenda:
A copy of your organizations IRS exemption letter-as well as completed
990 Form Documentation (ex. meeting of the minutes) that the proposal
is supported by organizations board of directors.
