Grant Requests

Your proposal is a critical document in securing the funding for your grant, and must include these important elements:

• Cover Letter - A brief description of your organization
• Background Information - History and purpose of your organization
• Specific Requests - Description of the proposed program and it's objectives
• Personal Information - Include the organizations personnel who will implement the program and their qualifications
• Financial Information - Include a detailed program budget
• Addenda - A copy of your organizations IRS exemption letter

Must submit six copies of grant request
